Job Description
Ready to spin the wheels of recovery right in patients’ homes? We’re hunting for a high-energy Home Health Physical Therapist to join our healthcare posse! You’ll be the groove-master who helps patients ditch pain, crank up mobility, and vibe their way to independence. With your knack for crafting personalized rehab jams, you’ll turn living rooms into recovery dance floors!
Key Responsibilities:
  • Be a movement mixologist: Assess patients’ physical flow in their home turf to uncover rehab needs and functional goals.
  • Drop tailored treatment beats with therapeutic exercises, manual therapy, and slick modalities to get patients moving like nobody’s watching.
  • Track progress like a pro, remix plans as needed, and keep the charts popping in electronic health records (EHR).
  • Hype up patients and their home crews with home exercise routines and slick injury prevention tips to keep the good vibes going.
  • Sync with our squad of physicians, occupational therapists, and home health aides for a chart-topping care experience.
  • Bust out cool moves like dry needling, kinesiology taping, or balance training to keep patients in the groove.
  • Stay chill with healthcare rules (we see you, HIPAA) and keep your skills fresh with continuing education.
  • Navigate patients’ homes safely, adapting to unique setups like a rehab ninja
Qualifications
  • Master’s degree in Physical Therapy.
  • Minimum of 1 year of experience in home health as a physical therapist.
  • Current physical therapy license within the state of practice, adhering to professional standards and practices.
  • Valid CPR certification, maintained throughout the clinical tenure.
  • Licensed driver with the ability to travel to various business locations.
Why is This a Great Opportunity
If you are passionate about making a tangible impact through compassionate care and developing meaningful therapeutic relationships, we invite you to apply and help our patients achieve greater independence and improved quality of life.
